Teen Center Registration Process

1. This facility is designed for teenagers between the ages of 13 – 17 years old.
2. All participants and their parent / guardian are required to complete a registration form and sign the Teen Center Pledge.
3. All participants and their parent / guardian must present Hamilton Township residency and Proof of Age for admission and participation in Teen Center activities.
4. Upon completion of items #2 & #3, a photo of each teen will be taken for the issuance of Hamilton Township Teen Center ID Card.
5. Hamilton Township Teen Center ID Card will allow teens access by way of the front door (only) during Teen Center operational hours (only).

Monitors—4
The Supervisor and Coordinator are adults who will manage and mentor in the Teen Center
Monitors will be younger (16—19 yrs. old) staff members who manage the main entrance, computer room and 2nd floor activity room.
